Non-urgent advice: Register as a New Patient
We are happy to register new patients who live within our practice area.
To register with the surgery please ask at reception and they will give you a new patient registration form to complete.
All new patients are required to have a new patient health check with our health care assistant. Please help us by attending the check-up as it enables us to get your up-to-date medical history.
If possible please register whole households at the same time.
You are free to see any doctor in the practice. We encourage patients to see the same doctor whenever possible to ensure that your care can be co-ordinated by that doctor. When you join the practice one of the doctors will be allocated to you as your usual doctor. Should you wish you may request that one of the other doctors becomes your usual doctor.
The practice catchment is defined as follows:
- TO THE NORTH: South side of Nile Street
- TO THE EAST: West side of Wilson Street/Paul Street/Provost Street
- TO THE SOUTH: South side of Chiswell Street/Ropemaker Street
- TO THE WEST: East side of Golden Lane/Central Street
In addition the area extends into the Golden Lane Estate and the roads surrounding St Bartholomew’s Hospital. Use the map below to check if you are within our practice area before registering.
Catchment Area
From January 2015, the Government provided GP practices with the option to register people at their practice who were not living in their practice area. City Road Medical Centre has decided to offer this new service as we believe that many people who commute into the City of London for work find it difficult to access GP services during the working day, which in turn can compromise their health and wellbeing.
We would require potential new patients to complete a pre-registration form prior to having their registration accepted. It is important to note that by registering with the City Road Medical Centre as an out of area patient, requests for home visits would need to be provided by the NHS services near to where an individual lives and would be accessed through ringing NHS 111.
Out of area registration is voluntary for GP practices, meaning patients may be refused because they live out of area.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
The medical centre is on the ground floor and has ramp access for wheelchairs and toilet facilities for the disabled.
We have a wheelchair that you can borrow to get to and from the practice. Please ask at reception.
